Broadlawns seeking Therapists and Social Workers

In addition, Broadlawns has primary and urgent care clinics in East Des Moines, Downtown Des Moines, and Dallas Center, and behavioral health support programs within the community.

Position:

We are seeking Therapists and Social Workers to join the Broadlawns Team in Outpatient Behavioral Health and in our Residential Group Homes.

Outpatient Behavioral Health

1. Functions autonomously in providing therapy services to clients with a variety of acute and chronic mental health conditions.
2. Works collaboratively with other members of the patient’s care team.
3. Conducts intake assessments as assigned.
4. Provides clinical consultation and educational programs to BMC staff and/or community agency program personnel as needed.
5. Maintains involvement with in-service training experiences.
6. Provides services off-site as needed.
7. Maintains current documentation of all clinical services provided in accordance with department and organization standards.

Residential

1. Conduct an initial psychosocial assessments and/or social history assessment for each new Residential Care Facility client within thirty days of admission for long term care or within first day of admission for transitional care; completes assessment documentation in a timely manner.
2. Obtains client’s written permission, establishes contact with appropriate people from the client’s personal support system.
3. Obtains client’s written permission, establishes contact with appropriate community support people including case manager. Coordinates necessary treatment planning meetings including for formulation of the Individual Program Plan (IPP), the semi-annual review and or the residential treatment plan for the Residential Care clients.
4. Attends team meetings and staffing; actively participates in development of the multi-disciplinary treatment plan for each client including formational of appropriate and measurable goals.
5. Meets regularly with clients to provide ongoing assessment as well as support and interventions appropriate to their goals.
6. Provides therapy to clients within group therapy context. Assist in the development of and participates in programming and activities appropriate to each client
7. Maintains current documentation of all services provided in accordance with department standards. Assist with training other staff regarding appropriate documentation to demonstrate services provided.

Qualifications:

• Master’s Social Work degree from accredited educational institution.
• Current Social LMSW, LISW, LMHC, or LMFT license with State of Iowa.
• Knowledge in crisis intervention techniques and brief therapy.
• Ability to work cooperatively with a multi-disciplinary team.
• Excellent written and verbal communication skills.
• Valid Driver’s License Required for Residential position.
