Position:
We are seeking Therapists and Social Workers to join the Broadlawns Team in Outpatient Behavioral Health and in our Residential Group Homes.
Outpatient Behavioral Health
- Functions autonomously in providing therapy services to clients with a variety of acute and chronic mental health conditions.
- Works collaboratively with other members of the patient’s care team.
- Conducts intake assessments as assigned.
- Provides clinical consultation and educational programs to BMC staff and/or community agency program personnel as needed.
- Maintains involvement with in-service training experiences.
- Provides services off-site as needed.
- Maintains current documentation of all clinical services provided in accordance with department and organization standards.
Residential
- Conduct an initial psychosocial assessments and/or social history assessment for each new Residential Care Facility client within thirty days of admission for long term care or within first day of admission for transitional care; completes assessment documentation in a timely manner.
- Obtains client’s written permission, establishes contact with appropriate people from the client’s personal support system.
- Obtains client’s written permission, establishes contact with appropriate community support people including case manager. Coordinates necessary treatment planning meetings including for formulation of the Individual Program Plan (IPP), the semi-annual review and or the residential treatment plan for the Residential Care clients.
- Attends team meetings and staffing; actively participates in development of the multi-disciplinary treatment plan for each client including formational of appropriate and measurable goals.
- Meets regularly with clients to provide ongoing assessment as well as support and interventions appropriate to their goals.
- Provides therapy to clients within group therapy context. Assist in the development of and participates in programming and activities appropriate to each client
- Maintains current documentation of all services provided in accordance with department standards. Assist with training other staff regarding appropriate documentation to demonstrate services provided.
Qualifications:
- Master’s Social Work degree from accredited educational institution.
- Current Social LMSW, LISW, LMHC, or LMFT license with State of Iowa.
- Knowledge in crisis intervention techniques and brief therapy.
- Ability to work cooperatively with a multi-disciplinary team.
- Excellent written and verbal communication skills.
- Valid Driver’s License Required for Residential position.
