Care Manager or Therapist needed: UnityPoint Des Moines
Care Manager- Powell Chemical Dependency Center, UnityPoint Iowa Lutheran Hospital, Des Moines, IA, Full time position with benefits
Overview: Integrates and coordinates care of individuals across the healthcare continuum. Facilitates the interdisciplinary plan of care in order to meet multiple service needs, promotes continuity of care through elimination of fragmentation of care/service, and facilitates the effective utilization of resources. Serves as educator and a central source of communication for the individual and their support systems.
Responsibilities: Completes a comprehensive initial assessment of the individual’s physical and mental health status including clinical conditions, support systems and resources to identify needs and make referrals to appropriate multi-disciplinary services. Documents initial evaluation in a timely manner. Provides telephone triage for persons calling in (potential waiting list patients, referrals to ED or Intensive Outpatient Services. Completes all insurance authorizations, utilization review for patient care and leave management documentation. Monitors and coordinates an interdisciplinary plan of care that is in partnership with the individual and their support services for needs and services across the health care continuum and for transition through the levels of care
Qualifications: Master of Arts/Science degree in a mental health, psychology, social work or human services-related field required. Must be licensed at one of the following levels: LISW, LMHC, LMFT, Masters or other Counselor/Clinician with independent licensure within 1 year of hire. Clinical experience related to adult substance abuse preferred. Must have IBC (Iowa Board of Certification – Substance Abuse) certification or receive certification within one year of employment.
Mental Health Therapist- Powell Chemical Dependency Center, UnityPoint Iowa Lutheran Hospital, Des Moines, IA, Full time position with benefits
Overview: Provides therapy to clients referred for outpatient and behavioral health services. Participates as a member of a multidisciplinary team to provide continuity of care for patients attending outpatient programs. Maintains accountability for own care to assure adherence to ethical standards. Consistently utilizes a holistic approach (considering physical, psych/social, spiritual, educational, safety and related criteria, appropriate to the age of the patients served in assigned service area.
Responsibilities: Provides individual and group therapy utilizing a variety of treatment modalities. Assumes responsibility for maintaining professional and clinical expertise. Provides education/information to patients, families and referral sources. Consistently utilizes a holistic approach (considering the physical, social/psychological, and educational level of patients, and families). Arranges referrals for spiritual, educational, safety and related needs of patients/families.
Qualifications: Master’s level graduate in counseling, social work, psychology, family therapy or related behavioral health field. Temporarily licensed as a master’s level mental health clinician in Iowa and completion of independent license within 2 years of employment. Temporarily certified as an addiction counselor in Iowa or completion within 1 year of employment followed by complete certification according to IBC guidelines. Preferred experience working with patients with alcohol/drug addiction.
Pay Range Commensurate with experience. This is an exempt position. Salary Minimum $25.02
